Patient Rights and Responsibilies

RIGHTS

As a patient at Student Health & Wellbeing Services you may expect:

  • Respect and courteous, considerate care regardless of your age, race, gender, religion, national origin or sexual orientation.
  • Privacy and confidentiality of treatment and medical/mental health information within the confines of the law.
  • To review or request photocopies of your medical and/or mental health records in accordance with Student Health Center policy.
  • To know who is treating you and, when possible, to a choice of health care provider.
  • To full information about your diagnosis, the effectiveness of treatment, possible risks, side effects or alternate methods of treatment.
  • To participate in decisions regarding your health care except when contra-indicated for medical and/or safety reasons.
  • To ask for a second professional (medical or mental health) opinion before making any decision.
  • If you decide not to be treated, you should be made aware of the medical / mental health consequences of your decision.
  • To prompt assistance in resolving complaints or grievances regarding your treatment.

RESPONSIBILITIES

As a patient at Student Health & Wellbeing Services you should expect to:

  • Present complete and accurate information about your health, including present and past illnesses and other matters pertaining to your health.
  • Keep appointments, be on time, and call to cancel an appointment as soon as possible so that other patients might utilize that time.
  • Pay for medications or fee-based services or supplies through your Humboldt account.
  • Comply with the treatment plan developed with your health care provider.
  • Pay a no-show fee ($30/medical or $20/CAPS counseling) which will be charged to your account when you do not show for your appointment and fail to cancel 24 hours in advance.
  • Ask questions when you do not understand what a clinician asks or informs you about your diagnosis or treatment. 
  • Designate another legally authorized person to assist you when needed to help you understand and make treatment choices.
  • Treat Student Health & Counseling staff and other patients with courtesy and respect according to the Humboldt code of conduct.
  • Provide a responsible adult to provide transportation home and to remain with you as directed by the provider or as indicated on discharge instructions.
