Immunization Requirements:

The California State University (CSU) is committed to the protection of health and wellness of all students. To comply with this overarching goal, CSU campuses implement procedures to ensure that students are educated about and receive immunizations to prevent potentially serious and contagious diseases. Immunizations, Screening Requirements, and Recommendations are adopted from the California Department of Public Health (CDPH) IMMUNIZATION & SCREENING RECOMMENDATIONS FOR COLLEGE STUDENTS.  Please AVOID A HOLD on your REGISTRATION and submit your immunization forms (including proof of vaccination/s, and/or any relevant exemptions, where applicable) prior to deadline.  Link:  Cal Poly Humboldt's Immunization Policy (updated March 2023).

Cal Poly Humboldt students are REQUIRED to obtain the following Immunizations & Screenings:

    • Measles, Mumps and Rubella (MMR): Two (2) doses with first dose on or after 1st birthday; OR positive titer (laboratory evidence of immunity to disease)
    • Hepatitis B (Hep B): Students age 18 and younger (CA Health & Safety Code, Sec. 120390.5)
    • Varicella (Chickenpox): Two (2) doses with first dose on or after 1st birthday; OR positive titer. History of contracting the disease does not suffice.

Additional Requirements for those that live in Cal Poly Humboldt Residential Housing:

    • COVID-19: Individuals must be fully immunized according to current recommendations of the CDC.    
    • Meningococcal conjugate (Serogroups A, C, Y, & W‐135): One (1) dose on or after age 16 for all students who are ages 16-21 years.

Additional Requirements for those who are engaged in Fieldwork or Clinical Placements 

All students enrolled in health care fields of study or teacher education programs are required to provide documentation regarding the completion of immunizations and health screenings as established by the state licensing agencies or clinical placement sites or fieldwork sites.  Students should contact the academic program for the specific required immunizations, health screenings (e.g., TB screening and physical examination) and any other fieldwork or clinical placement requirements.

Exemptions:

A student may be exempted from any rquirements in this policy as described below.

    • Medical Exemptions apply when a medical condition for which an approved vaccine presents a significant risk of a serious adverse reaction prevents you from being immunized.
    • Religious Exemptions apply when: a) You have a sincerely held religious belief, observance, or practices, which includes any traditionally organized religion, or b) You hold beliefs, observances or practices that occupy a place of importance in your life (comparable to that of traditionally recognized religious) that are in opposition with receiving the immunization.

How & When to Submit your immunization records 

We encourage submission of records at the very start of the semester of admission to allow time to resolve any issues.  The final deadlines are as follows:

    • Deadline for those entering in the Fall: September 20
    • Deadline for those entering in the Spring: February 20

Complete the Immunization Checklist by either:

    • coming to the SHC and completing a hard copy of the questionnaire

Upload a copy of your immunizations through the Secure Health Portal as instructed on the questionnaire.

Go to the Health Portal:

    1. Document Upload tab and select Immunization Records for the Document type.
    2. Upload a picture of your immunization records showing your name, date of birth, and clinical documentation.
        • If uploading a photo of your covid vaccination card, choose "Covid Vaccination Card" as the document type

If you have a hold on your student account due to immunization noncompliance, and you submit materials meeting all requirements, your hold will be lifted in 24-48 hours of submission. If you are a new student, please submit your questionaire checklist and any relevant records before the to-do list item becomes a hold on your class registration for your second semester at Cal Poly Humboldt. 

If you are submitting a medical EXEMPTION to a required immunization, note this on your immunization checklist and ask your healthcare professional to complete the below form. Medical exemptions cannot be granted without this documentation.

Vaccines available at the Student Health Center

    • MMR (Measles, Mumps, and Rubella): Required Immunization

    • Hepatitis A: 2 shots over 6 months, recommended for travel
    • Td (Tetanus and diphtheria) and Tdap (adult Tetanus, diphtheria and pertussis): Required Immunization; Should be updated every 5-10 years
    • Some vaccines may be available for individual ordering, such as Meningococcus B, HPV, Typhoid or Rabies. Please inquire at the Health Center

NEW! The Public Health Department of Humboldt County is now providing low cost immunizations for adults that are either uninsured or underinsured with their VFA program (vaccines for adults). Potential recipients are required to fill out a screening form to assess eligibility. Under the VFA program the following vaccines are available at low cost: HPV, MMR, Tdap, Varicella, Hepatitis B, Menactra (Meningitis ACWY), in addition to several others.

Public Health accepts Medi-Cal or Partnership Health in Humboldt County.  When you call to get scheduled for a vaccination, make sure to ask about lower cost pricing if you believe you qualify.

Local Pharmacies:

Vaccinations are also available at local pharmacies and they are able to bill insurance.  Consult your local pharmacy for more details.

Please note, although not required, the following immunizations/screenings are recommended (students are encouraged to discuss this with their provider):

    • Tetanus, Diphtheria and Pertussis (Tdap): One (1) dose after 7 years of age. 
    • Tuberculosis Screening/Risk Assessment (TB): Students are strongly encouraged to complete a questionnaire and any follow-up requirements.
    • Hepatitis A (Hep A): All students regardless of age.
    • Hepatitis B (Hep B): Students age 19 and older.
    • Human papillomavirus (HPV): Women and men through 26 years of age.
    • Influenza (Flu): Annually for all students regardless of age.
    • Meningococcal B (Meningitis B): Students age 16 – 23 who elect vaccination after discussion with their healthcare provider.
    • Meningococcal conjugate (Meningitis): Students age 16 to 23 years of age.
    • Pneumococcal: For students with certain medical conditions (e.g., severe asthma, diabetes, chronic liver or kidney disease)
    • Poliovirus (Polio): Regardless of age, if the series was not completed as a child.
    • Immunizations for international travel based on destination.

Tuberculosis (TB) testing at the Student Health Center:

As part of the immunization recommendations for all Cal Poly Humboldt students, completing the TB screening form in the Health Portal is highly recommended. Depending on the answers submitted on this form or other circumstances, we can perform TB testing during immunization hours. We offer TB skin tests and, if needed, IGRA blood tests (Quantiferon Gold). TB skin tests are a cost of $10 billed to the student's account. For more information on TB: https://www.cdc.gov/tb/. We strongly encourage you to complete your immunizations with your primary care provider prior to coming to Cal Poly Humboldt. 

Your primary care provider will be able to bill your insurance. However, if you are unable to do the the required immunizations before you start at Humboldt, the vaccines can be obtained at the Health Center for a fee. Fees for immunizations will be billed to your student account. We do not bill insurance. A hold will be placed on your ability to register for classes unless the health center has copies of your immunization records. Please note that even if you gave them to Admissions as part of your transcripts, you still need to file them with the health center.

Frequently Asked Questions about Immunizations/Holds on Registration

Why do I have a hold on my registration?

If you do not provide your immunization status in the 2 step process described above, including proof of vaccination against Hep B, a registration HOLD will be placed on your account. The registration HOLD will be removed only after the records are received at the Student Health Center. How do I check to see if my records have been received?

If you have mailed or faxed your records in, please allow two weeks for processing and it will be marked as complete. If you use the quick online method available at https://health.humboldt.edu/Document to upload a picture of your records as instructed, the requirement will be marked complete if all requirements are met and

the hold will be removed in 1 to 2 business days. Depending on the method, you can check your task list to see if the item has been marked complete after the time has elapsed, and if the hold still appears contact the Health Center by calling (707) 826-3146 or emailing health@humboldt.edu.
